Hurt Your Back Lifting the Couch or Cleaning? What to Do About a Pulled Back Muscle

The man in the photo knows this feeling. Saturday morning, cleaning day. You bend to shift the couch so you can sweep under it, or lift a heavy bucket of water, or carry the gas cylinder to the kitchen. Then a sharp catch or pulling feeling grips your lower back. You freeze, one hand on your back, and straighten up slowly. You manage the rest of the day, but by evening your back is stiff. Days later, it still nags when you sit at work, roll over in bed or bend to wear your shoes.
This is one of the most common back injuries we treat in Westlands, and the good news is it usually recovers well with the right care.
What happened to your back
When you lift something heavy, especially while bending and twisting, the muscles and tissues of your lower back take a sudden overload. Small fibres get strained, and the muscles around them tighten to protect the area. That's the sharp pull you felt, followed by the stiff, nagging ache.
It happens more easily when you're not used to heavy lifting, when you lift fast, or when your back is already tired from a week of sitting.
What to do in the first few days
- Keep gently moving. Short walks around the house help more than lying in bed.
- Use heat. A warm towel or hot water bottle for 15 to 20 minutes eases tight muscles.
- Avoid heavy lifting and twisting for now, but keep doing light daily tasks.
- Sleep comfortably: on your side with a pillow between your knees, or on your back with a pillow under your knees.
When it's time to see a physiotherapist
Book an assessment if the pain hasn't eased within a few days, keeps you from sleeping or working, comes back every time you lift, or keeps nagging when you sit.
How we treat it at Katasi Physiotherapy
We check exactly where your back is strained and how it moves. Hands-on therapy, massage and dry needling relax the tight, guarding muscles, and many people feel looser after the first session. Then we give you gentle exercises to get your back moving and build strength, and show you how to lift safely so it doesn't happen again on the next cleaning day.
If your back has fully locked up, read Back Muscle Spasm: What to Do When Your Back "Locks". If pain runs down your leg, read Sciatica: When Pain Runs From Your Back Down Your Leg.
Lift smarter next time
Get close to the load, bend your knees, keep your back long, and turn with your feet instead of twisting. For a couch, ask for help.
See a doctor straight away if
You have numbness or weakness in your legs, loss of bladder or bowel control, numbness around your private parts, fever, or the pain followed a fall.
