Woke Up With a Stiff Neck? How Your Pillow or a Couch Nap Caused It

woke up with a stiff neck — woman holding her painful neck in bed after sleeping on a hard pillow, Katasi Physiotherapy Westlands Nairobi

You fell asleep on the couch halfway through a movie, head propped on the armrest. Or you spent the night on a hard, high pillow at a friend's place or a hotel. Now, like the woman in the photo, you wake up and reach for the side of your neck. It's tight, sore and stuck. Turning your head to check your phone, reverse the car or look over your shoulder sends a sharp pain down the side of your neck. When you press the muscle, it feels hard as a rope.

Why your neck got stuck

Your neck muscles hold up your head, which weighs about as much as a bowling ball. When you sleep with your neck bent or twisted for hours, one side of your neck is stretched and the other is squeezed. Those muscles hold that awkward position all night with no break.

By morning, they've tightened up to protect your neck, almost like a cramp that won't let go. That's why the muscle feels hard and painful to touch, and why some movements are blocked.

Common causes

  • Falling asleep on the couch with your head on the armrest or slumped forward
  • A hard or high pillow that pushes your head up and sideways
  • A flat, worn-out pillow that lets your head drop
  • Sleeping on your stomach with your head turned to one side all night
  • A long, tiring day at the laptop, so your neck muscles were already tight before bed

What to do today

  • Use heat: a warm towel or hot shower on the tight muscle for 15 to 20 minutes
  • Keep moving gently: slowly turn and tilt your head within comfort, a few times every hour
  • Avoid a neck collar or keeping your neck completely still; that makes it stiffer
  • Tonight, fix your pillow: on your back, use one pillow that supports the curve of your neck; on your side, the pillow should fill the gap between your shoulder and ear so your head stays level

When to see a physiotherapist

Most stiff necks ease in a few days. Book an assessment if yours lasts more than a few days, keeps coming back, gives you headaches, or spreads into your shoulder or arm.

How Katasi Physiotherapy treats a stiff neck

We check which muscles are tight and how your neck moves. Then we release the knots with hands-on therapy, massage and dry needling, gently free the stiff joints, and give you simple stretches and strengthening exercises. We'll also help you choose the right pillow and sleeping position. Many people turn their head much more freely by the end of the first session.
